Referral form to alert first responders of behavioral health needs
Typically through completing and submitting a form that includes identifying information, people can notify law enforcement of a family member’s conditions and needs–including mental illness–to better inform personnel for potential future encounters.

Chester County, PA Chester Police Department
- People with special needs and their families can fill out a form, called the Premise Alert, to alert first responders of their needs.
- The completed form is submitted to person's local police department, and the information is entered into the county's 911 center database
- The system started in Chester County in 2004 and has since expanded into a statewide initiative.
